Two iOS recorder fixes.

Re-arm the input on resume(), match the lock order in stop() to the configuration change path so the two cannot deadlock, and clean up the adapter node on a configuration change (#1295, @mdydek, closes #1294)
Keep writing to the same open file after an iOS configuration change, and give files opened within the same second a segment number so their names cannot collide (#1296, @mdydek)
